目的 汉化老年人复杂性自评量表(The INTERMED for the elderly self-assessment,IM-E-SA),并在老年慢性病患者中进行信度、效度检验,以期为医护工作者测量患者的复杂性提供评估工具。方法 2023 年 8 月取得作者授权,2023 年 11月采用改良 Brislin 翻译模型对 IM-E-SA 量表翻译、回译及跨文化调适。采用便利抽样法,于 2024 年 8 月至 11 月选取贵阳市某三级甲等综合医院的 405 例老年慢性病患者对汉化后形成的量表的信度、效度进行检验。结果 405 例老年慢性病患者完成研究。中文版老年人复杂性自评量表共 20 个条目,4 个维度,包括生物、心理、社会、医疗保健各 5 个条目。总量表的内容效度指数为 0.993,Cronbach α 系数为 0.865,重测信度为 0.883、折半信度为 0.615,各维度 Cronbach α 系数为 0.766~0.879,重测信度为 0.812~0.890。探索性因子分析 KMO 值为 0.832,Bartlett’s 球形检验值为 2001.388(P<0.001),累计方差贡献率 64.560%。结论 中文版 IM-E-SA 量表信度、效度良好,可用于国内老年慢性病患者中复杂性识别。
Objective To translate and culturally adapt the INTERMED for the elderly self-assessment(IM-E-SA)into Chinese and test its reliability and validity among older adults with chronic diseases,so as to provide healthcare professionals with an instrument for measuring patient complexity. Methods Having been authorised in August 2023,a modified Brislin translation model was used to translate,back-translate,and cross-culturally adapt the IM-E-SA in November 2023. Then a convenience sampling was employed to recruit 405 elderly patients with chronic disease from a Tier-IIIA hospital in Guiyang,between August to November 2024, to test the reliability and validity of the translated and adapted version of IM-E-SA. Results All 405 patients completed the study. The Chinese version of IM-E-SA comprised 4 dimensions with a total of 20 items:biology(5 items),psychology(5 items),society(5 items), and healthcare(5 items). The content validity index of the scale was 0.993,Cronbach's alpha coefficient was 0.865,test-retest reliability was 0.883,and split-half reliability was 0.615. Cronbach's alpha coefficients of the dimensions ranged from 0.766-0.879 with testretest reliabilities from 0.812 to 0.890. The Kaiser-Meyer-Olkin( KMO)value was 0.832 and Bartlett's sphericity test yielded a value of 2001.388(P<0.001),accounting for 64.560% of cumulative variance. Conclusion The Chinese version of the IM-E-SA has good reliability and validity and can be used to identify complexity in elderly patients with chronic disease in China.
