目的 探讨基于二元疾病管理理论构建的二元应对干预方案对抑郁症患者及其配偶二元应对水平、亲密关系的影响。方法 采用文献回顾、专家会议结合患者的特点及临床实践经验,基于二元疾病管理理论构建二元应对干预方案。采用便利抽样法,将 2023 年 9 月— 2024 年 6 月本院心理科住院的 48 对抑郁症患者及其配偶作为研究对象,采用随机数字表法分为干预组(n=23)和对照组(n=25)。对照组患者及其配偶给予常规护理,干预组患者及其配偶在此基础上实施二元应对干预方案。比较干预前及干预 8w 后抑郁症患者及其配偶的二元应对水平、亲密关系、沟通模式和心理健康素养。结果 抑郁症患者及其配偶的二元应对水平、亲密关系、建设性沟通、心理健康素养得分干预前后差值高于对照组,差异具有统计学意义(均P<0.05)。结论 基于二元疾病管理理论的二元应对干预能够有效提高抑郁症患者及其配偶的二元应对水平、改善患者及其配偶关系、优化患者及其配偶沟通模式以及提升患者及其配偶心理健康素养水平,本研究干预措施为抑郁症的家庭协同干预提供了新思路,具有临床推广价值。
Objective To examine the impact of a dyadic coping intervention program based on the dyadic illness management theory on dyadic coping capacity and intimate relationship between the patients with depression and their spouses. Methods Based on dyadic illness management theory,a dyadic coping intervention program was developed according to the results of literature review, expert consultation,clinical experience,and patient characteristics. With the convenience sampling method,a total of 48 patients with depression and hospitalised in our hospital and their spouses between September 2023 and June 2024 were recruited,together with their spouses. Patient-spouse pairs were randomly assigned to an intervention group(n=23)or a control group(n=25). The pairs in control group received routine nursing,while those in the intervention group received the dyadic coping intervention program in addition to the routine nursing. The Chinese version of the dyadic coping inventory(C-DCI),short marital adjustment and prediction test(MAT), the communication pattern questionnaire(CPQ)and multicomponent mental health literacy(MHL)were used to compared the effect of intervention before and 8 weeks after intervention. Results The scores difference before and after intervention for dyadic coping, intimacy,constructive communication and mental health literacy of the patients and spouses in the intervention group were significantly higher than those of the control group(all P<0.05). Conclusion The dyadic coping interventions developed on the basis of dyadic disease management theory can effectively enhance the dyadic coping capacity among the patients with depression and their couples,improve marital relationship,optimise couple communication patterns and enhance their mental health literacy. The dyadic coping interventions developed in this study provide insight into family-based collaborative interventions for depression with clinical promotion value.
