2023 年 12 月广东省护理学会正式发布《老年糖尿病患者居家管理指导规范》团体标准（T/GDNAS 029-2023）（以下简称《标准》）。《标准》聚焦在专业评估的基础之上制定个性化方案，内容包括血糖管理指导、用药管理指导、日常管理指导、支持与转介 4 个方面。《标准》采用文献法、专家研讨法与德尔菲法函询编制，旨在为各级医疗与医养结合机构的具备执业资格护士开展老年糖尿病患者居家管理指导实践工作提供指引，规范护理操作。本文将对《标准》的主要内容进行解读，旨在精准地把握居家管理指导的关键环节。

In December 2023，the Guangdong Nursing Association published a group standard of Guidelines for home management of elderly patients with diabetes mellitus（T/GDNAS 029-2023）. On the basis of systematic professional assessment， the Standard provides individualised care across four domains：glycaemic management guidance，medication guidance，daily self-care management and support/referral. It was developed through an integrated process of systematic literature review，expert panel discussion and Delphi consultation. The document aims to standardise nursing practice and guide the registered nurses in hospital and institutions of integrated medical and elderly care when delivering home-based management for older adults with diabetes. This interpretation synthesises the Standard’s core content to facilitate precise implementation of the key procedures.
